Overview
The Pathways to Success (P2S) team will collaborate with youth and young adults (YYA) in their home communities to develop a Plan of Action (POA) aimed at reaching their individualized goals such as: completing high school, independence, and continuing education or career. Together, the P2S team and YYA will assess the following areas of possible need: housing, crisis planning, family, basic needs, employment, education, health, and spiritual/community connectedness to develop innovative individualized plans with YYA. These POAs seek support from local community resources, both formally and informally.
Referrals
Referrals are accepted from all counties! We welcome referrals from all entities as well as self-referrals from youth and young adults experiencing homelessness. Prioritized focus is being provided to the following counties: Divide, Williams, McKenzie, Rolette, Towner, Cavalier, Benson, Ramsey, and Eddy. This prioritization is due to the numbers of identified youth and young adults through various data points.
