Secondary Transition: Stepping Towards the Future is a series of webinars developed by Pathfinder Services of ND.

As a student with a disability enters their high school years, additional planning is needed to transition into life after high school. A well-planned, comprehensive and coordinated effort between the school, student, family, and appropriate post-school agencies, provides an opportunity for the student to step forward through a smooth transition process. This three-part series provides an overview the Individuals with Disabilities Education Act (IDEA) secondary transition requirements and how these requirements are incorporated into the Individualized Education Program (IEP) for students ages 6-21 in ND public schools.

The secondary transition assessment process involves a process of collecting data on a student's strengths and needs, preferences, and interests. Part 2 of Secondary Transition: Stepping Towards the Future reviews the components of a comprehensive secondary transition assessment and takes a closer look at the first three steps in the secondary transition IEP framework: measurable postsecondary goals; the Present Level of Academic and Functional Performance; and Special Considerations.

Secondary Transition: Stepping Towards The Future (Part 3)

In Part 3, we continue Stepping Towards the Future by completing the journey through the remaining sections of the ND secondary transition IEP framework. These sections include: anticipated coursework for the student's remaining high school years; graduation expectations; Transfer of Rights to the student; and the development of a coordinated set of strategies or services needed by the student including decisions about agency responsibilities and timelines.
